Dear Professor Fox

Thank you for forwarding on your comments regarding the electronic
transmission form, which have been sent on to me.

As per your suggestion, we have now revised the form to include a comments
section (at the bottom just before the "signature"). I have attached the
revised version for you:

I understand that you have also had problems with pasting material in from
elsewhere. I have sought some advice on this (as I am not an expert on
Word!) and the following has been suggested - that you try selecting a
field by clicking on it once and then pressing Ctrl -V. If this doesn't
work, may I suggest you contact Markus Rossi directly (mrossi@wiley.co.uk)
and he should be able to help you further.

I just filled in electronic version of Transmission form
It was quite nice but here are a couple of suggestions
1) I am naive at word, but as I used it I could not paste information in
   One gets errors and takes more time because of this
   I typically have addresses, authors etc. in another window for cut and
paste
2) I suggest a place for notes and comments
