1 2. If you receive a message about invalidating authorization, click OK if you do not have Lotus Notes Mail IDs at your site; otherwise, click Cancel
2 3. If you see a list of subforms, click Cancel
3 4. Create fields, static text and other elements on the form
4 5. Choose Properties Design | Form to assign a name and other form properties
5 6. Close and save the form
