	This claim is for my son Oliver Fox who is covered  by Blue Cross/Blue Shield
	Contract Number XJJ215529736 (I enclose a Card Copy)
	
	Blue Cross initially rejected claim but when I queried Insurance representative, I
	found that they had incorrect information on file about my family. (I just
	moved here from Syracuse)
	I resubmitted claim several months ago and until I got your note, assumed it
	had been paid. 
	I think it is best if Crouse bill Blue Cross again!
