The Faculty Handbook says:
"Each faculty member shall prepare a written report on research and service accomplishments at the end of each semester to document performance of non-instructional assigned activities. It will be necessary for all units to ensure that the documents are submitted by all faculty, contain information about performance in all areas of assigned duties, and are in an easily audited format. Many units have designed special forms for submitting "Evidence of Performance."
The EOP document is very important because the data contained in it is used to evaluate both the individual faculty and the department. It is audited by the State for compliance with the "12-Hour Law", checked against PARS forms for auditing faculty time reporting data, and used by the Provost in making decisions regarding departmental allocations of budget, faculty lines, and other resources. Therefore, failure to submit the information on time is sufficient cause for an expression of official concern on an individual's annual evaluation.
The following is our department's special form for submitting the Evidence of Performance. To reduce duplicative paperwork, the department chair is asking you to submit information using the this form only once a year (end of Spring term). The other semester (end of Fall term) you provide the information in the format required by the department's Faculty Evaluation Committee (FEC).
Please notice that the form has changed. This is not the department's fault. The changes are to reflect new requirements put on the department to provide data to the Provost and Board of Regents.
You are required to include all the items specified, or indicate "none" if you have no evidence for a particular item this year. A few sample items of information are included, by way of example. You are expected to remove these and replace them with your own information.
The data is only for a one year period starting with May and ending with the following April. The example is for the period May 1999 through April 2000. You are expected to replace internal references to 1999-2000 (and other years) as appropriate to fit it to the current year.
The department chair regrets that we cannot have a single form for both EOP and FEC vitae, since the content of the FEC folder it is not sufficient for the 12-Hour Compliance files and is not broken down into the categories required by the Provost. Until such time as the FEC vita format is revised, everyone is encouraged to organize data within the FEC vita in a way that permits simply cutting and pasting the relevant items into the EOP document.
